PRIVACY ACT 2020 & CREDIT REPORTING PRIVACY CODE 2004

TO: COMMUNITY FINANCIAL SERVICES LTD T/A CFS MONEY

RE: APPLICATION FOR FINANCE

1. I/We:
   1. Declare the information given in respect of the attached application is true and correct and that I am / we are not an undischarged bankrupt/s.
   2. Agree you may collect from credit reporting agencies, credit providers, including Ministry of Justice (MoJ) fines, my/our employer/s accountants, or other names person/s or relative/s, the personal, financial and commercial information and references about me/us as is necessary for the purpose of considering this application. And I/we agree that you may produce this authority to such parties for the purpose of collection of such information.
   3. Agree that any information recorded by credit reporting agencies may be passed to others seeking information from these said agencies in regards to my credit. This may consist of credit inquiries, payment defaults but not limited to these solely under the Credit Reporting Privacy Code 2004. All information recorded will be for the purposes related to the provision of credit to me.
   4. Agree to meet the cost of an independent valuation of the security to support this application such valuation to be obtained by you and also I/We agree to pay all legal costs reasonably incurred by you in consequence of this application. My/Our solicitor/agent is authorised to accept notification on my/our behalf.
2. I/We acknowledge that if CFS provides this finance CFS may from time to time collect and monitor personal information about me/us as they consider advisable for the administration and protection of such agreement between us. This may include further credit inquiries and/or credit monitoring services for purposes related to the provision of credit to me.
   
   I/We authorise CFS to communicate with me by way of electronic format (including but not limited to email or texting).
3. I/We understand that CFS and any of its related companies may send me/us details of other products and services from time to time. The term “related company" includes any member of the CFS group of companies.
4. I understand and authorise that from time to time CFS may disclose my personal information or disclose my name, address and phone number to another agency, approved by CFS for the purpose of market research, offering additional credit facilities, merchandising, or other services. 

Where the personal information may readily be retrieved, I/we shall have access to it and the right to request correction.
